Know Your Coverage

Understand My Policy

Upload your insurance policy document and our AI will help you understand what's covered, what's excluded, and find useful clauses for appeals.

Upload Your Policy Document

Tip: Upload your Summary of Benefits and Coverage (SBC) or Medical Policy document. These are typically available from your insurance company's website or your HR department.
Select the type of document you're uploading.

Upload your document (PDF, DOCX, or TXT — max 20MB).
Training use: Your document will be stored encrypted and may be used in de-identified form to train and improve our AI models, helping us better assist patients fighting insurance denials.
Knowing your plan type helps us identify the right regulator and appeal rights. Your plan type affects who oversees your appeals:
ERISA employer plans → Dept. of Labor • ACA Marketplace → HHS / State Insurance Dept. • Medicare → CMS • Medicaid/CHIP → State Medicaid Agency • Not sure? Select "I'm Not Sure" and we'll try to determine it from your document.

Your Information

This form goes to our server. We store your policy encrypted, with a scrambled version of your email so you can ask us to delete it. If you join the mailing list, we keep your name and email to send updates. We don't sell or share your personal information. We may use uploaded documents, with identifying details removed as best we can, to improve our AI and help other patients.

Terms & Agreements

Help other people fight back

Share a copy of these documents with us. We learn from them what care costs, how costs and coverage change over time, and what needs approval first, and we share what we learn so others can fight back.

If you tick the box, you agree to share a copy of these documents, and what you've told us about the plan, with Fight Health Insurance. We combine them with documents from other people to learn how plans work, and we share what we learn publicly. AI models read them, including ones we run and outside services. We keep what you share as long as we can. Please do your best to leave out your name, member ID and health information, unless you want to include them. Either way, we do our best to remove personal details from anything we share. If you ask us to delete your data, we do our best to delete this copy too. What we've learned about the plan stays, because it describes the plan, not you.

What does sharing mean?

Ticking the box saves a separate copy of these documents for research, with what you've told us about the plan. It changes nothing else here. If you leave the box unticked, we make no copy.

We keep the files encrypted. Our team and AI models, including outside services, read them with documents from many other people, and we share what we learn publicly. If you ask us to delete your data, we do our best to delete your files, including this copy. What we've learned about the plan, like what kind of plan it is, stays, because it describes the plan, not you.
