Explanation of Benefits (EOB)

An explanation of benefits is the statement your plan sends after processing a claim, showing the amount billed, the allowed amount, what the plan paid, and what you may owe. It is not a bill, but it tells you whether a claim was paid or denied and why. Reading the EOB carefully is usually the first step in spotting an error worth appealing.
